Summary:	本文旨在探討全民健保對台灣老人居住安排的影響。全民健保的實施提供老人部份醫療支出的補助，降低未來醫療風險與成本的不確定性，因而產生間接的所得效果，在老人經濟狀況更獨立自主下，生活費用越不用依靠子女的奉養，是否造成獨居或僅與配偶同住機率會顯著上升？本文採用衛生署國民健康局「台灣地區中老年身心社會生活狀況長期追蹤調查」共五年的Panel資料，將調查資料中所有受訪老人依全民健保實施年度區分成1995年前有健保者及1995年前無健保者兩個族群。在全民健保實施後，因1995年前有健保者醫療風險並無改變，所以將其視為對照組；1995年前無健保者轉變成有健保，會受健保制度實施的影響，因此將其視為對照組。本研究採用「差異中的差異」（Difference-in-Difference）方法，以Random effect Probit機率模型估計全民健保實施前後對台灣老人獨居或僅與配偶住機率的影響。實證結果發現，全民健保政策效果對老人獨居機率並無顯著影響，雖然老年人在全民健保上獲得醫療支出補助佔可支配所得的比例頗高，但這並不是主要構成改變其居住安排的誘因。在受到傳統觀念影響下，兒子個數的多寡為影響老人居住型態的主因；此外，當受訪者已婚且配偶建在、擁有資產或有工作、越偏好獨居或僅與配偶住及長子女有偶或有工作下，老年人越會傾向獨居或僅與配偶住。 The purpose of this paper is to investigate the effect of the National Health Insurance (NHI) on living arrangement of the elderly. The NHI offers substantial medical subsides to the elderly who were previously uninsured and reduce uncertainty about future medical risks and costs, which makes the elderly more economic independent. As a result, the probability of the elderly living alone or with spouse is expected to increase. We use data from the Survey of Health and Living Status of the Elderly in Taiwan to exam whether the elderly will be more likely to live alone or with spouse after the NHI. After NHI was implemented, the respondents who had health insurance before 1995 are viewed as control group whereas the respondents who did not have health insurance before 1995 are treated as treatment group. We use Difference-in-Difference and Random effect probit model to estimate the Effect of the National Health Insurance on the probability of the elderly living alone or spouse.We found that the NHI has no significant effect on the probability of living alone or with spouse although the elderly obtain substantial medical subsides from the NHI. The tradition of living with the son is the main determinant of the living arrangement of the elderly. Moreover, an individual is more likely to live alone or with spouse if he/she is married and the spouse is still alive, is currently working, has financial assets, shows strong preference to living alone or with spouse as well as if his(her) first child is married or currently working.
