    摘要註: 台灣近年來都市化的生活使人運動習慣減少,伴隨肥胖人口的增加,糖尿病已成為台灣主要死亡原因之一,因此如何有效控制糖尿病或是改善糖尿病之併發症是必要的課題。關於運動介入對於糖尿病患之研究文獻已經不少,但探討低強度和低容量運動介入對第二型糖尿病患之作用,相關研究顯少,故本研究目的為探討規律且適合第二型糖尿病患之運動介入,對第二型糖尿病患作用之影響。研究方法:採用實驗研究法進行研究,使用結構型問卷調查第二型糖尿病患之運動阻礙及疾病史後,20位運動組受試者進行12週低強度(35-54% MHR)、低容量(每週運動消耗1,200kcal)之快走運動介入,並收集運動介入前、後之受試者total cholesterol、HDL-C、triglyceride、fasting immunoreactive insulin、fasting plasma glucose、glycosylated hemoglobin及HOMA-IR等相關數據,以t-test及simple linear regression analysis進行分析(P<0.05),研究低強度與低容量之運動介入能否改善第二型糖尿病患之血液生化值及胰島素抗性,另有20位對照組受試者未進行運動介入。結果顯示,有12位(57.7%)受試者運動阻礙因素為無多餘運動時間,10位(25%)有高血壓疾病史,32位(80%)有糖尿病家族疾病史;經12週的運動介入後,運動組受試者其胰島素抗性呈顯著降低,其他血液數值變化雖無顯著性差異,但呈現改善趨勢;此外,12週的運動介入前後胰島素抗性的降低,與空腹胰島素呈顯著相關,與其餘血液值之間則無顯著相關。對於行動能力尚可的糖尿病患者而言,於忙碌的生活中妥善規劃時間,利用低強度、低容量的持恆運動方式,以降低其胰島素抗性並促進血糖代謝趨穩定,甚至改善病情會是較容易且可行的方式。 The urbanization of Taiwan has led the people to have a sedentary life style, and accompany with the increasing of the obesity population. Diabetes mellitus has become one of the main causes of death, therefore, controlling or improving the complications of diabetes mellitus is an essential issue. There have been lots of researches regarding exercise intervention in the diabetic patients, yet the researches regarding effects of low-intensity and low-volume exercise intervention in type 2 diabetes patients are still quite few.Consequently, the purpose of this research is to probe into the regular and appropriate exercise intervention for type 2 diabetes patients, as well as its related effects.Methodology: this research used the experimentation method, to investigate the exercise barrier and the disease history of the diabetes patients by valid questionnaire. Twenty subjects of exercise group implemented 12 weeks of low-intensity (35-54% MHR) and low-volume(1,200 kcal consumed per week) exercise intervention by brisk walking while another 20 subjects of control group did not proceed any exercise intervention. Total cholesterol, HDL-C, triglyceride, fasting immunoreactive insulin, fasting plasma glucose, glycosylated hemoglobin, and HOMA-IR before and after exercise intervention were analyzed by t-test and simple linear regression analysis (P<0.05), to examine whether the low-intensity and low-volume exercise intervention is able to improve diabetes patients' biochemical values and insulin resistance.The results show that, twelve subjects (57.7%) have exercise barrier of "lack of time", ten subjects (25%) have history of "hypertension", and 32 subjects (80%) have family medical history of "diabetes mellitus". Subjects in the exercise group significantly reduced the insulin resistance after 12 weeks of exercise intervention, and the other biochemical values showed improvement, though without statistically significance. Furthermore, the reduction of insulin resistance after exercise intervention is significantly correlated to fasting immunoreactive insulin, while the other biochemical values presented no significant correlations.For diabetes patients having passable mobility, reducing insulin resistance to stabilize the plasma glucose metabolism, and even to improve patients' condition by regularly implementing low-intensity and low-volume exercise intervention, is a simple and feasible method.
