    摘要註: 目的:本研究目的在評估台灣南部某些基層診所糖尿病患,進行為期2年的個案管理及飲食衛教後,評估其生化代謝指標及飲食攝取改變之成效。方法:將所招募之第2型糖尿病患,以糖化血色素≧7%之第2型糖尿病患進行個案管理,共有134人納入分析。隨機分派成介入組(n=63 ),及對照組(n=71 ),於介入第1年、第2年進行評值,每3-6個月執行個案追蹤及糖尿病自我管理教育課程,並將資料完整者94人進一步分析生化及飲食衛教成效。結果:在接受個案管理中自我足部檢查項目(n=134),在初診後評值或介入第1年、第2年介入組自我足部檢查完成比例都顯著高於對照組(p<0.001)。在成效分析部分(n=94),二組糖化血色素再介入後第1年(7.9±0.9%vs8.9±1.8%, p=0.001)與第2年(8.5±1.1%vs8.1±1.2%, p=0.071),介入組糖化血色素皆較控制組為低。在糖化血色素於介入第1年或第2年變化量在二組間之差異,介入組在第1年糖化血色素之改善情形較控制組佳(p=0.042),但在第2年之改善情形,在二組間則無顯著差異。在熱量與營養素攝取介入後第1年二組比較,熱量、醣類及蛋白質受取在介入組顯著低於對照組(p<0.05);在介入第2年的比較,只有熱量攝取部份,介入組低於對照組,但僅達邊緣性顯著差異(p=0.062)。結論:糖尿病的個案管理及飲食衛教介入一年後對糖化血色素及營養素攝取(醣類、蛋白質)皆有顯著改善之影響,但介入二年後,此成效可能逐漸減弱。 Objective: The aim of this study was to evaluate effectiveness of diabetes care management and nutrition education after 2 year intervention in primary care in southern Taiwan, to assess biochemical metabolic and dietary intake indicators of changes.Methods: The recruitment of HbA1c≧7% of Type 2 diabetes patients for case management, included 134 diabetes patients, randomized into intervention group (n=63), and the control group (n=71), involved in the 1 years in the 2 years of the evaluation value, every 3-6months to perform case tracking and diabetes self- management education programs, and 94 people with complete data and further analysis of biochemical and nutrient education effectiveness.Results: In the case management project of self-foot examination after the first visit, first year and two years, self-foot examination in the intervention group was significantly better than the control group(p<0.001). Comparing the HbA1c between two groups, patients in the inthervention group had lower HbA1c level in the first year (7.9 ± 0.9% vs. 8.8 ± 1.8%, p= 0.001) and the second year (8.1 ± 1.2% vs. 8.7 ± 1.5%, p= 0.071). In the changes of HbA1c from baseline status to status after one year or two year intervention, improvement was significant better in the intervention group than control group in the first year evaluation (p=0.042) but the significance disappeared in the second year. For the nutrient intakes, intervention group had significantly lower intakes of total calorie, carbohydrate and protein than control group after one year (p<0.05). However, only the total calorir intake was lower in the intervention group than control group after two ars and the difference was only at borderline significance (p=0.062).Conclusion: Diabetes case management and nutrient education intervention diet after 1 year have a significant impact on improvement in HbA1c level and nutrient intake, but this effect may be rebound after two year.
