摘要註:	近年來,臺灣醫療機構有M型化之趨勢,醫學中心與區域醫院為達規模經濟,朝多角化策略經營,基層院所則多以專科為主;然而地區醫院規模則處於不大不小的尷尬局面,使其面臨是否繼續經營的抉擇。因此,本研究利用2003年至2012年的全民健康保險資料庫,以實證研究探討在城鄉差距,也就是在是否有醫學中心、居家護理、是否為公立醫院等相關影響條件下,地區醫院若朝向多角化經營,其經營成效是否有所差異。實證研究結果顯示,地區醫院在都市中,其多角化程度的效果不如市郊來的好。此代表著,位在市郊中的地區醫院,若強化其多角化經營,亦或與鄰近居家護理合作,其經營成效會較好。因此,從銀行授信角度而言,可因位於市郊的地區醫院多角化程度,或者有與鄰近居家護理合作下,當作是否通過授信的依據之一。另外,也檢驗出地區醫院多角化的內生性影響因子為,是否有醫學中心和是否有居家護理的影響。 Recently, the trend of the medical institutions in Taiwan becomes M-form. Medical centers and regional hospitals who manage towards diversification strategies achieve economies of scale, but physician clinics & dental clinics are more of a specialization. However, the size of local community hospitals are between the size of teaching medical centers and clinics. Therefore, the management of local community hospitals need to make the decision of the diversification strategy. This study is an empirical study to examine the diversification strategy of local community hospitals by using the National Health Insurance database from 2003 to 2012. The empirical results show that the degree of diversification of local community hospitals in cites is not as effective as the ones in the rural areas. This represents that local community hospitals in the rural areas or nearby nursing home are benefited with diversification. The results can provide bankers as the reference of the credit management to the local community hospitals. Also, the endogenous factors of diversification of local community hospitals are whether there are medical centers and whether are nursing homes.
