摘要註:	根據健保局統計，目前全民健保存在「看病多、拿藥多與檢查多的三多問題」，看病多部份，每年國人平均就醫次數持續攀升，從1997年的13.76次，增加到 2012年的15.12次；拿藥多方面，每年健保門診藥費持續攀升，從2001年的688億元，增加到2012年的1185億元，占健保支出4分之1，較其他國家偏高。由於醫學科技進步，現代人平均壽命愈來愈長，年齡愈大，罹患一種以上疾病的機率就愈高；加上醫療科技發展，藥物、診斷、檢查等相當普遍。鑑此，我國近年來積極地推行電子病歷政策，2010年初行政院衛生署更首開先例地推出了「加速醫療院所實施電子病歷系統計畫」，由政府出資補助醫院實施電子病歷，急速升溫了國內醫院與資訊業者對實施電子病歷關注的熱度。其中，將優先補助與醫院民眾切身息息相關的醫學影像報告、血液檢驗、門診用藥記錄及出院病歷摘要四大項目，希望鼓勵國內各級醫院實施電子病歷、推動電子病歷互通，讓醫院透過資訊科技應用，能打破資源、地域、時空限制，加速病患診療決策，提昇醫療服務品質，減少醫療資源浪費，邁向院際間醫療資訊之整合，院際互通之電子病歷的願景。本研究在國內各級醫院在電子病歷互通的趨勢中，針對去過醫院診所的民眾進行影響電子病歷互通的使用意願七大假說，隱私的干預、聲譽、資訊敏感度、特定對象的資訊隱私關注、信任信念、認知的風險、認知上的效益進行調查，調查結果去醫院的民眾本身認知電子病歷互通的效益，其次是對醫院的信任信念，而信任的評估主要來自醫院診所的聲譽，取代了以往電子病歷互通上最重視的隱私問題。 Bureau of National Health Insufance reported that patient taked more treatment, medicine and examination than they needed.For examples, average treatments per patient increase from 13.76% times to 15.12 since 1997 to 2012, and total medicine expense increase from NTD 68.8 billion to 118.5 billion from 2001 to 21012 and total medicine expense is equal to 25% total health expense that is higher than other countries.Nowadays, people get more than one disease because of the increase of average lives, additionaly, the growth of medical technology, medicine, diagnosis and examine is normal. Therefore, Taiwan do hard work about the policy of electronic medical records . In the beginning of 2010, the Heath bureau has be the pioneer of 『 Increasing hospital electronic medical records. In the plan』, the fund is come from the govement, and it let the hospitals and comparies focus on electronic medical records. In the plan,medical image report, blood examination,medicine use records and medical history are the most important issues,and hope that the eneourage of the mutual use of electronic medical records can break the limit of resourse, time and region, increase the patiend diagnosis speed, pomote the quality of medical service, decrease the naste of medical resource. Most importantly, reach the goal of medical information combination and mutual use of electronic medical records between hospitals.In 2010, Health bureau support hospital implement electronic medical records, and want to descrease total health expense.However, the willingness of the mutual use of electronic medical records is one of the key success factors to implement.
