Summary:	多重抗藥性微生物(Multi-drug-resistant organisms；MDROs)與日俱增之現象已成為當今全球性公共衛生之重要議題。世界衛生組織(World Health Organization；WHO)因此將「抗生素抗藥性(antimicrobial resistance)」定為2011年4月7日世界衛生日(World Health Day)之主題。WHO 並且強烈呼籲各國政府採取有效的抗生素管理政策以遏止抗生素抗藥性之擴散。政府之醫療衛生政策及醫療照護體系在對抗生素抗藥性採取有效之介入措施上，扮演關鍵性的角色，而政府對抗生素問題癥結的了解與遏止承諾是必要的。因此我們應用管理學的方法，包括態勢分析法(SWOT analysis)；PEST分析法(PEST analysis)；價值鏈分析法(Value Chain Analysis)及五力分析法(Five Forces Analysis)來分析與回顧台灣抗生素抗藥之相關政策，並以“藍海策略(Blue Ocean Strategy)” 來擬定台灣抗生素管理策略，以有效控制抗生素抗藥性細菌之形成與傳播。研究結果發現，有效的抗生素管理計畫需要全方位之管理方法和策略，包括政策與法規之訂定與執行，以及對教育和監視導入適當之資源。另外，由感控醫師對處方開立者採取具有主動介入與回饋措施功能之抗生素使用預期審核制度，配合醫院評鑑與查核及健保局的審查制度，能有效加強抗生素的管理，減少抗生素不當使用。 Multi-drug-resistant organisms (MDROs) are increasingly recognized as a global public health issue. The World Health Organization (WHO) has chosen antimicrobial resistance as its theme for World Health Day 7 April 2011. WHO strongly recommends that governments conduct antibiotic stewardship for containment of antimicrobial resistance.Government health policies and the health care systems in which they are implemented play a crucial role in determining the efficacy of interventions to contain antimicrobial resistance. National commitment to understand and address the problem is prerequisite. We use the methods of management including SWOT analysis；PEST；Value Chain Analysis; Five Forces Analysis to analyze and review the antibiotic resistance related policies in Taiwan；and adopt “Blue Ocean Strategy” to draft antimicrobial stewardship program to control effectively antibiotic resistance and spreading in Taiwan. Based on the research, effective antimicrobial stewardship program requires comprehensive management methods and strategies including the introduction and enforcement of appropriate regulations and allocation of appropriate resources for education and surveillance. Prospective audit of antimicrobial use with direct interaction and feedback to the prescriber, performed by an infectious diseases physician through hospital accreditation and inspection and national health insurance payment system can effectively result in reduced inappropriate use of antimicrobials.
