Summary:	本研究針對金門地區98-102年空中緊急醫療轉診情形所做的回溯性個案研究，探討包含轉診基本資料、轉診品質、轉送安全之分析及轉診後續追踪。研究發現，每年平均轉診81人。平均年齡為50.92歲(50.92±26.54)；61歲以上轉診人數為263人(64.61%)最多，以心臟血管內科疾病為主，男性多於女性，男女均以心臟血管內科疾病最多。轉診疾病主要是循環系統疾病234人(57.49%)最多，外傷及中毒之外因補充分類84人(20.64%)居次，以科別分內科188人(46.19%)佔最多、外科178人(43.73%)居次、婦產科20人(4.91%)第三、小兒科21人(5.16%)最少。金門醫院101年度6月神經外科醫師介入，發現102年神經外科轉診減少為7人，佔當年緊急轉診數88人之7.95%。此結果是緊急神經傷害在地化照顧的典範表現。追踪轉診北榮290病患平均天數19.45±4.34天，住院超過14天共有110人(38%)。住加護病房共計210人(76.56%)，轉診二週死亡35人(12.07%)，追踪轉診一年死亡人數共50人，佔轉診北榮病患之17.24%，有82.76%的病患可以一年以上的存活率，也表示空中緊急醫療轉診是有效的，並且有其必要性。98-102年直升機轉診總花費時間在3小時28分至5小時02分，若要有安全及快速縮短到3小時的高品質空中轉診，應由固定翼醫療專機轉送，而平常該醫療專機在「金門機場待命」，全部轉送過程在接到轉送通知後「3小時內」完成。 This is a retrospective study about Kinmen air emergency medical referral from 2009 to 2013, investigating the referral base information, quality, safety and follow-up.Our study found that average referral 81 patients every year, and average age is 50.92 (50.92±26.54). The most is group above-61-years, 263 (64.61%)patients, male more than female. The cardiovascular medicine disease is most, and main diagnosis is Circulatory system 234 patients (57.49%), and then is XVIII Supplemental Classification 84 patients (20.64%).For divisions, the most is medical department (188 patients, 46.19%), next is Surgical department (178 patients, 43.73%), and then Obstetrics and Gynecology (20 patients, 4.91%), the least is Pediatrics (21 patients, 5.16%).Our research found that 2012 neurosurgery patients reduced to 7 (7.95%), total patient in 2012 is 88, because of the intervention of neurosurgery doctors in June 2012. This result is a display of urgent nerve damage in localized care.Followed-up 290 Taipei Veterans General Hospital patients were hospitalized Average 19.45±4.34 days, 110(38%) of those were hospitalized exceed 14 days. 210 patients (76.56%) were hospitalized In ICU, and 35 (12.07%) patients passed out in referral two weeks; 50 (17.24) patients were dead in referral one year. The most important is 82.76% referral patients could live exceed one year, this represent air emergency medical referral is effective and necessary.Referral time from 2009 to 2013 was about 3 hours 28 minutes to 5 hours 2 minutes.If we want referral to be in 3 hours high quality and safety, it should be transferred by fixed wing medical airplane which standby in Kinmen.
