Obesity is a growing global health issue, particularly among reproductive-aged women, increasing risks during pregnancy such as gestational diabetes, hypertension, and preeclampsia. These complications affect both maternal and fetal health and can have lasting consequences. A major concern in obese pregnancies is metabolic dysfunction, including lipid and glucose dysregulation. While cholesterol is vital, excess levels raise cardiovascular and oxidative stress risks. Elevated lactate and insulin resistance, assessed by HOMA-IR, are common in obesity and worsen during pregnancy.Body composition significantly influences pregnancy outcomes-greater muscle mass and lower fat levels enhance insulin sensitivity and energy. Regular exercise improves body composition, regulates lipids, lowers lactate, and boosts insulin sensitivity, reducing obesity-related risks. Maternal exercise also benefits offspring, lowering the child's risk of metabolic disorders like dyslipidemia and type 2 diabetes by improving maternal glucose control and influencing fetal epigenetics.Emerging research shows maternal exercise reduces inflammation and supports healthy fetal growth. This study investigates how prenatal exercise affects infant body composition and metabolic biomarkers. We hypothesize that aerobic exercise during pregnancy lowers infant adiposity and improves insulin and lipid profiles.Healthy pregnant women (<16 weeks gestation) were randomized to supervised moderate-intensity exercise (aerobic, resistance, or combined) or a control group. Maternal and infant body composition was measured, and blood biomarkers were collected. Findings from Aim 1 showed that higher weekly exercise volume and longer duration were linked to lower maternal weight and improved biomarkers, particularly with aerobic exercise.Aims 2 and 3 demonstrated that maternal exercise influenced infant body composition and lipid profiles. Aerobic exercise was associated with lower non-HDL and higher HDL cholesterol in infants, while resistance training reduced LDL cholesterol. Greater exercise frequency, intensity, and volume were linked to improved infant growth and metabolic health, emphasizing the importance of structured prenatal exercise for optimizing early-life outcomes.
