Summary:
自民國八十四年健保制度實施,使得國人在醫療資源運用及健康意識觀念有所改變,以往因經濟壓力不敢看病與濫用成藥等現象,逐漸被醫療資源過度使用而取代,現今醫療體系與以往大不相同,許多醫院已經朝向集團化與企業化經營,有組織性的行政架構與標準流程、計畫性地實施醫療策略聯盟合作,且更為注重管理與行銷對醫院經營的影響力。台灣全民健保於民國九十一年起開始實施總額預算制度,對所有醫療院所而言,健保局的總額預算制度侷限健保醫療業務的成長,健保資源早已無法滿足國人日趨重視的預防保健概念,其中地區醫院於成本控制與人力資源在經營管理上相較於其他醫療院所處於弱勢。經由SWOT分析、對個案醫院的資深員工作深入訪談,希望透過本研究之結果、結論與建議盼能提供給地區醫院之管理及領導者一些因應當前健保政策之經營管理方向,使得在激烈的競爭環境下,地區醫院得以創造永續生存發展空間。 After the implementation of National Health Insurance in 1995 in Taiwan, the use of medical resources in the country started to change, in which patients’ resistance to receive further treatments due to economic pressure was replaced by over-use of medical resources. Also because of the implementation, many hospitals transformed themselves to be a part of bigger business group and became more business-oriented as they now have more organized administration system and standard processes, more strategic alliances with each other and paid more attention to hospitals’ management and marketing capabilities. In 2002, the global budget payment system was introduced to the National Health Insurance. This system not only limited the development of hospitals in general but also placing district hospitals in a weak position in comparison to others. With reference of these situations, by using SWOT analysis and interviews with the case hospital’s senior employees, this study aims to offer some advices for hospitals faced with similar circumstances and to help district hospitals to grow sustainably in a competitive environment.
