Summary:	摘要本研究主要探討全民健康保險對罹患慢性病的中老年人的快樂和道德風險的影響,當罹患慢性病的中老年人,在所得減少和健康惡化的情況下,面臨醫療費用的上升而可能產生經濟壓力以及心理方面的影響。此外,全民健康保險的實施會降低罹患慢性病中老年人的醫療成本,因此對中老年人的快樂產生影響;但較低的醫療成本也可能產生道德風險現象,像是有抽菸、飲酒或是嚼食檳榔等行為的中老年人可能會因為健康保險的實施而提升這些不健康行為。本研究採衛生福利部「中老年身心社會生活狀況長期追蹤調查」共五年的追蹤資料,將罹患慢性病的中老年人區分為1995年全民健康保險實施前,有健康保險與沒有健康保險的中老年人,無保險者視為實驗組,有保險者視為控制組,並以difference-in-differences分析全民健康保險實施前後,對罹患慢性病的中老年人的快樂與道德風險的影響。實證結果顯示,全民健康保險對罹患慢性病的中老年人的快樂與道德風險並無顯著影響。 This paper examines the effect of the National Health Insurance (NHI) on the happiness and moral hazard of the elderly with chronic diseases. The NHI reduces the costs of the medical care, which is particularly important for the elderly as their health deteriorates and income declines over time. On the other hand, the implementation of the NHI lower medical costs potentially cause the problem of moral hazard because the out-of-pocket medical expenses are lowered by the implementation of NHI. We used five-wave of panel data from the Survey of Health and Living Status of the Elderly in Taiwan, 1989-2003 for the analysis. The sample is then divided into treatment and comparison groups based on whether respondents had health insurance before 1995. The results from the difference-in-differences estimation show that the implementation of the NHI has no significant effect on happiness and moral hazard among the elderly with chronic diseases.
